I am a patient here just passing through. In the past 15 months I’ve been to 5 different Skilled Nursing Facilities. As a whole, this one has been the best. One problem, no one is allowed to enjoy the two, gated and eclosed, beautiful courtyards in the front. I spoke with the director and he told me that the back patio is for patients without visits to safely enjoy fresh air and the summer sun. I’m writing this because I just spent 10 minutes trying to hold the door open to this back patio helping a frustrated handicapped resident in a wheel chair who could not open nor could he get over the hump in the doorway. This leads me to believe that this back patio is a deterrent to keep residents from ever going outside. That passage way will not pass any legal A.D.A. standard or regulations. Can this be adjudicated or is going outside to far fetched?

Every time I stop by here I just have to say the reception is is real nice with a smile on her face its nice to see it when having a ruff day thank you

This place is horrible. My grandpa passed way here around 2009-2010. Everyone we would come to visit, his teeth were not brushed in days, he hadn’t showered in days, & they failed to properly turn him over that he developed really huge bad bed sores on his back. What are these people getting payed for if they’re not doing their job. No human being deserves to be treated this way. My last memories of my grandpa are sadly him not in the best condition due to neglect. Every time we would visit, my mom would have to do all the work. The relief my grandpa would get when my mom would finally clean him up, is just so sad he had to live like that. This place just sucks. It always stuck in there, always dirty, & the elevator was scary to ride. This place shouldn’t even be in business. It should be ran by someone who truly cares about the health & well-being of loved family members. But of course they don’t truly care.

My Dad was recently here for his post-hospital stay, and I cannot say enough about the amazing team here and how well they took care of him. The nursing and physical therapy teams were great, case management was on it and very communicative (shoutout to Kassandra, Adriana, and their team) for a seamless handoff. Before we even got my pops home, home health had reached out to get him scheduled. Just an absolute wonderful experience with this entire care team <3

First time here I’m recovering from a blood disorder, excellent nurse and CNA coverage per patient. The food is fine, there’s lots of activities to get involved with and a wonderful physical therapy/ workout room.

I am deeply disappointed with the conditions and care at this rehabilitation center. The facility was extremely hot inside when it should have been air conditioned, creating an uncomfortable and unsafe environment for patients. The floors were sticky and unclean, which is unacceptable for a healthcare facility. Even more concerning, my friend—who is currently a resident—was left sitting in a dirty diaper for a long period of time. A nurse claimed the change had been done, but it had not, and her daughter ultimately had to come in herself to change her. This is not only unprofessional but also neglectful. With the amount of money being paid for her stay, the level of care should be far better. Residents deserve dignity, cleanliness, and basic professionalism. Families should not have to step in to do the work that trained staff are being paid to provide. I would strongly advise others to carefully reconsider before placing a loved one here.

I’d give zero stars if I could. It is filthy as soon as you walk past the front desk. The rooms are totally unkept with disgusting furniture, they also have bedbugs. I immediately told the staff and they were completely unfazed. Do not send your loved ones here

During my stay all CNA were very sweet and attentive especially Rosalinda. Housekeeping was also very friendly and kept my room very clean. Prescy was also very kind and caring to all patients. I also want to point out how quick and helpful the social services department was during my stay they were on top of everything I had questions about.

I am so impressed with the care my dad received at this nursing home. The nursing staff provided excellent care and were always attentive to his needs. The admission process was smooth and easy, and the team made sure everything was in place for his stay. Andrea and Katrina made us feel seen and gave us the one on one time to explain the process of a skilled nursing stay. When it came time for discharge, the process was seamless, and they worked with us every step of the way. Special thanks to the therapists who helped with my dad’s rehab—thanks to their hard work and dedication, he’s now back at home, and we couldn’t be more grateful. Truly an outstanding experience from start to finish

The staff members are genuinely kind and patient, and it’s clear that they truly care about the residents they care for. The building is clean, and there is a strong sense of teamwork among the staff. Whenever I had a question or needed assistance, Harmony was an excellent resource and consistently went above and beyond. What stood out to me the most was how personal everything felt—residents are not just viewed as “patients” here. They are treated with respect and warmth.

My uncle was there from around January-May 2025. I will share my honest thoughts. First, the food is terrible, disgusting and really shouldn’t be fed to human beings. Thankfully, my family was able to combat this by taking turns brining him food daily. However, I do feel that this did negatively impact his quality of life. I also feel like it is negatively impacting the lives of all the residents there. All humans are deserving of quality food. I am aware that this is a administrative issue, not a staff issue. Secondly, there are two or three people in each room and it is extremely tight. I am aware again that this is no one persons fault. My uncle was total care 100% needed assistance for every aspect. I am sure that is was tough on the staff at times caring for someone who was in poor condition like my uncle. I feel as though the staff did the best they could do for him. He always looked clean, and so did his bed. They changed him daily. His changes were painful for him, but that couldn’t be avoided. I never saw them be rough with him, and he never complained about the way they treated him. They always asked for permission / explained what they were doing when they would handle him. Anytime he would hit his call button, it seemed they would come as fast as they were physically capable. I didn’t expect for someone to come running in right away, that is unrealistic. The front desk people were a little slow at acknowledging my presence sometimes but it wasn’t terrible and they always did accommodate for what I requested. Overall, I feel as though it’s an average nursing home, that provides normal expectations. The residents are able to free from the hallways as well as outside.

This facility is in serious need of a proper inspection and major staffing improvements. To begin with, the social services department is extremely difficult to communicate with. Reaching a social worker is next to impossible, and when visiting in person, I’ve encountered staff who seem frustrated and unmotivated—as if clients are the reason for their unhappiness. On several occasions, I’ve witnessed patients outside the building complaining about being ignored or mistreated, which is deeply concerning. Collaboration and communication across departments are severely lacking. Clients are discharged without clear information or updates about where they’re being sent or what the plan is. This lack of transparency not only creates confusion but also jeopardizes continuity of care. In addition, phone support is unhelpful. Calls are often placed on hold indefinitely without any real effort to connect you with someone who can help. It feels like the system is designed to deflect rather than support. This facility urgently needs new leadership and staff who are committed to making a positive impact in people’s lives—individuals who take pride in their work and understand the importance of empathy and communication in healthcare. On a positive note, I will say that the nursing staff has consistently provided better customer service than the social workers or front desk staff. They are courteous, attentive, and seem genuinely invested in patient care. On my end I will be rapport all these issues to ombudsman or any other agencies I can , tired of seeing these injustice and putting the incompetent of the staff.

Our experience with Country Hills has been great. The staff is incredibly kind and treats each resident with patience and respect. I would like to also mention that the physical therapists and their assistants are outstanding. They take time to understand each patients needs and provide care. I would send my loved ones here again

This place is freaking horrible. My poor aunt is there, soon to be gone. She has been complaining about a rash for weeks. No one has done a thing for her. Come to find out the rash is from bed bug bites. Place is dirty. The food is disgusting. Anyone at anytime has access to your room & bathroom, it’s ridiculous. I’m going to be putting in a formal complaint.

The worst place ever to have your family member in . They don’t clean the place it stinks the nurses are more worried about about how big they’re but looks in the scrubs they wear and always have an attitude when you ask questions . It really sucks to see places like this one open
